Acute Renal Failure

Impression: ARF / Uremia
Condition: Fair – Poor
Diet: Low salt / Diabetic (DM)
Activity: As tolerated
Vital Sign: As routine
•    Consider any dehydration manifestation
Please:
1.    CBC, BUN, Cr, Na, K, UA
2.    UA ( pyuria, hematuria, cast … )
•    Correct hyperkalemia
3.    VBG (Metabolic acidosis)
4.    Hydrate with Normal saline 500-1000 ml over 1-2 hrs.
5.    Internal Foley
6.    Check Intake / Output
7.    KUB Sonography ( R/O Obstructive causes )
•    Urology consult (if needed/hydronephrosis)
•    D/C NSAID, Aminoglycoside, Contrast and any nephrotoxic
•    Adjust other medication (as anephric dose)
